Cash-pay direct primary care clinic in a suburban Raleigh, North Carolina office suite: self-employed families paying $75–$99 monthly memberships for unlimited visits
Problem: Hard-to-get, expensive primary care under insurance with long waits and high deductibles · Offer: Monthly membership for unlimited primary care visits
The idea has real money signals and a workable unit economics estimate, but a near-identical competitor (doctor direct) already covers about 75% of the offer, and the market size data is for a much wider market than the idea, so the case rests on untested assumptions.
No data, left out of the score: demand
Doctor direct overlaps
The closest competitor already offers about 75% of the idea. Uniqueness is the weakest axis, so the concept must find a difference that buyers can name.
Price sits in a band
Competitor prices run 50–150 USD a month with a median of 75, so the proposed $75–$99 membership sits inside the observed band. Price is context, not a signal, until buyers show they will pay it.
Availability gap open
Two complaints about availability and minimum order were found, and no competitor addresses them. One complaint is an anecdote, but this gap is the clearest opening for the concept.14
Free search channel open
The Google query for primary care without insurance has no direct competitor in its top 3 results, so a free traffic source exists.
Ten rivals crowd field
Ten direct competitors were found, so buyers have many options and the idea must stand out.15161718192021222324
Market size unmeasured locally
The published market size covers a much wider market than the idea, so there is no data on the size of the suburban Raleigh segment itself.10
Unit economics look sound
The category-norm estimate shows a 45% margin and a 2-month payback at 75 USD a month. This is not measured in this market and must be checked.
Model has failed before
Qliance, a similar direct primary care model, closed, which shows the model can fail even when demand exists.9

What the idea covers
- ›Price — A flat monthly fee with no insurance deductibles addresses cost complaints directly.
- ›Availability, minimum order — Membership promises access, but the data does not state wait times or appointment capacity.
- ›Service and support — Direct access to a physician suggests better support, but no support model is described.
- ›Quality — The idea says nothing about clinical quality measures or outcomes.
- ›Paperwork and payment — Eliminating insurance billing removes much paperwork for patients, though it is not stated explicitly.
The customer's voice
the annual costs for membership alone would be thousands of dollars
given how much trouble I've had getting appointments, and how it just took me five months to find a new PCP who would take me at all
Warning- caution do not enter into an agreement with MDVIP unless you are ready to pay the full amount for a year even if you cannot use their services for the entire year.
The individual MDVIP does not provide a backup physician when they go on vacation.
The doctor did not listen to anything I asked for and required unnecessary tests.
I signed up for one in January, but so far it's been a five-figure sum to get an email newsletter.
